Required Information for enrollment:

​Youth 
ANNUALLY:  Youth Enrollment, Medical/Health Release, Media Release, Code of Conduct

Adult Volunteers
ANNUALLY:  Adult Enrollment, Risk Managment training
FIRST YEAR OF VOLUNTEERISM AND EVERY 3 YEARS THEREAFTER:  In addition to enrollment and Risk Management training: Volunteer Service Application, Background Check, 4-H Volunteers Online Modules, UAF Transportation training (if needed)

Insurance for 4-H Activities
​Insurance documents for special events and claims should be kept on file for 3 years.
